Mrs. Wong insists on living in her home of 35 years even though none of her children live in Washington State. Worried about her increasing forgetfulness and frailty, her children enrolled her in the Community Care Network of Kin On. A personal care assistant now helps Mrs. Wong with her daily needs and meals while her children coordinate Mrs. Wong's care with the Kin On Chronic Care Manager through email and telephone.

After a stroke paralyzed his right side, Mr. Chen came to live at the Kin On Health Care Center where he receives speech and physical therapy. Instead of being homebound and isolated, Mr. Chen participates in the special activities with other residents. He also enjoys watching the fish in the aquarium, reading the Chinese newspapers, and visiting with his children and grandchildren.
